How to book ?

Option (A) : come to our Registration Counter on 11/12/2016 , between 12pm -4pm at 6th Floor, Wisma MCA, Jalan Ampang.

** bring a copy of NRIC, 1 passport size photo, 1 full length photo & RM250 ( for divorcees/widow/widower pls bring relevant document to proof) 

Option (B)  : (i) Download Application Form at www.cupidspacedating.com

 (ii) fill in the form

(iii) contact us at 012-2401802 to refer commissioner for oath for Statutory Declaration for single status

(iv) Whatsapp the completed application form to us at 012-2401802 for  approval and make payment
